An accident at work causing the death of an employee. An accident at work causing an employee to be unable to carry out their normal work for more than three consecutive days, not counting the day of the accident but including weekends and rest days. An accident to a person who is not an employee, caused by the work activity, that requires treatment by a registered medical practitioner or admission to hospital. The dangerous occurrences listed in Schedule 1 of the Regulations, such as scaffold collapse, the failure of lifting equipment, an unintended explosion or fire, or a release of a substance that could cause serious injury, whether or not anyone was hurt.
Fatalities are reported to the HSA immediately by telephone and followed by a written report; the scene is left undisturbed until the HSA and the Garda allow otherwise. Other reportable accidents and dangerous occurrences are reported within ten working days of the event, using the HSA online reporting system. Where the absence only becomes more than three days after the fact, the ten days run from when the employer learns the threshold has been passed.
